Quest Diagnostics Lab Savings Program*

The Lab Savings Program is a voluntary program that provides benefits for covered lab services with no out-of-pocket cost*. This optional program provides covered, non-emergency outpatient lab services for zero deductible and zero coinsurance when Quest Diagnostics Inc.** does the testing.

To obtain 100 percent laboratory benefit coverage, you MUST present your MHBP ID card when specimens are collected, and all lab testing must be performed by Quest Diagnostics. In addition:

• Tell your doctor you want Quest Diagnostics used for the lab processing.
• If your doctor doesn't already use a Quest Diagnostics service facility, information about one in your area is available at 1-800-377-7220, or search for a lab near you from this site.
• Quest Diagnostics will perform your laboratory tests, and your claim will be processed automatically without you submitting it.
• You won't file paperwork or pay an out-of-pocket cost!

The Lab Savings Program is an optional program. You're free to use any lab you choose, inside or outside the Coventry National Network, and receive regular benefits for covered lab services.

For your convenience, print this handy reminder and give it to your doctor or provider the next time you need to have covered lab work done.
